Cialis boosts sex after spinal cord injuries

The impotence pill Cialis appears to work even in men with spinal cord injuries, French researchers said on Monday.

Impotence often follows spinal cord injuries. Only about 25 percent of men with such injuries are capable of having sex, Dr. Francois Giuliano and colleagues at the Raymond Poincare Hospital in Garches, France said.

They found that Cialis tripled the number of times the men could have sex.

Their study, funded by Eli Lilly and Co, maker of tadalafil or Cialis, involved 197 men with an average age of 38 in France, Germany, Italy and Spain with spinal cord injuries.

After a one-month waiting period, in which no one got treatment, a questionnaire to assess sexual function found both groups had moderate erectile dysfunction , Giuliano's team reported in the Archives of Neurology.

Then 142 men were assigned to the Cialis group and 44 got a placebo for a 12-week period, taking no more than one pill daily as needed before sexual activity.

After 4 months, men taking Cialis were successful nearly half the time they attempted intercourse, while men in the placebo group succeeded only 16.8 percent of the time.

Cialis and similar drugs work by increasing blood flow to the genitals.

The researchers said the Cialis study achieved success similar to that found in studies of Pfizer Inc.'s Viagra or sildenafil and Glaxosmithkline Plc's Levitra or vardenafil, all of which improved erections in men with impotence after spinal cord injury.

€1 million spent on Viagra-like drugs in HSE West

LIMERICK CITY and county has the highest public spend on Viagra and other erectile dysfunction drugs per head of population in the HSE West area.

Figures released by the HSE show that spending on erectile dysfunction drugs topped €1 million through the medical card system for the first time in 2008 in the HSE West area.

HSE West forum member Cllr Brian Meaney (Green) called on the HSE yesterday to reduce the monthly “allowance” on the drugs to public patients.

The figures show that the spend on erectile dysfunction drugs, including Viagra and Cialis, increased by 6 per cent in 2008 in the HSE West area, which stretches from Donegal to Limerick.

A sum of just over €1.021 million was spent on the drugs in 2008 – an increase of €63,771 on the €957,320 expenditure in 2007.

Public patients are restricted to purchasing four Viagra-type tablets per month by the HSE. Each prescription costs about €25, along with a dispensing fee to the chemist.

The figures show that most of the people receiving the drugs were in the 55-64 age category, accounting for 30 per cent or €306,584 of the spend in 2008.

In the breakdown provided, the figures show that those aged in the 65-74 age category accounted for 25 per cent or €258,205 of the spend, while those aged 45-54 accounted for 22 per cent or €226,750.

Viagra and Cialis are the market leaders, with Viagra being the most popular drug accounting for 57.4 per cent of the spend. Both drugs regularly feature on the HSE’s top 100 most commonly dispensed drugs.

The figures show that the spend on the erectile dysfunction drugs in Limerick represents 22.7 per cent of the total spend in the HSE West area despite having only 18 per cent of the region’s males aged over 24, according to 2006 Census figures.

Some 19.9 per cent of the spend on erectile dysfunction drugs was made in Co Galway; 14.8 per cent in Co Donegal; 11.5 per cent in Co Mayo; 10 per cent in Co Clare; 6.5 per cent in Sligo/Leitrim; and 6.2 per cent in Roscommon. An additional 8.4 per cent of overall spend went to the Limerick/East Tipperary area.

The public spend per head of the male population aged over 24 years of age in Limerick was €3.9 compared with €2.7 in Galway and Clare. The per capita spend was €3.1 in Roscommon; €2.8 in Mayo; €2.4 in Co Donegal; and €2.2 in Sligo/Leitrim.

Nationally, the spend on Cialis and Viagra, the two most popular drugs in 2008, increased by 6 per cent from €2.2 million to €2.4 million.

Yesterday, Cllr Meaney said: “Across the HSE West area, the service is facing significant cutbacks this year and I don’t see why patients receiving erectile dysfunction drugs should be immune from that, and a reduction of four tablets to three per month sounds reasonable. That could save the taxpayer €250,000 per annum in the HSE West area.”

The HSE has said that Viagra is also prescribed for the treatment of Pulmonary Arterial Hypertension (PAH), adding that “it is not possible to quantify how much of the expenditure on medications on the information provided is for treating erectile dysfunction or Pulmonary Arterial Hypertension or other conditions”.

A team working for Pfizer, the pharmaceutical firm behind Viagra , in Sandwich, Kent, has uncovered a substance that boosts female sexual arousal, increasing her chance of orgasm.

The findings could pave the way for a drug similar in effect to Viagra, which has helped countless men.

In the study, using a novel prototype drug, researchers discovered more about the mechanisms underlying female sexual arousal.

The finding is published in the British Journal of Pharmacology.

The boffins found that electrically stimulating the pelvic nerve increases blood flow to the genitalia, and that this effect was enhanced if they also gave a prototype drug (UK-414,495).

They believe that the drug acts by blocking the breakdown of an internal chemical messenger that plays a key role in increasing blood flow during sexual arousal.

When women become aroused, blood flow increases to the vagina, labia and clitoris. This causes the organs to swell, and the vagina to relax, as well as increasing vaginal lubrication and the sensitivity of the genitalia.

Female sexual arousal disorder (FSAD) affects up to 40percent of women irrespective of age. These women find that their genital organs do not respond to sexual stimulation, they find arousal difficult and this causes them to become distressed.

"Before this work, we knew surprisingly little about the processes that control all of these changes," says the lead researcher in the project Chris Wayman. "Now we are beginning to establish the pathways involved in sexual arousal scientists may be able to find ways of helping women who would like to overcome FSAD."

In the study, researchers stimulated the pelvic nerve and measured changes in genital organs.

They believed the genital arousal occurred because stimulation of the nerve triggered the release of vasoactive intestinal peptide (VIP), a well-known neurotransmitter. VIP has only a short-lived effect, because it is soon broken down by an enzyme called Neutral Endopeptidase (NEP).

The researchers believe that their prototype drug increased the arousal because it blocked NEP's ability to break down VIP, therefore letting the VIP have a more powerful and prolonged effect increasing arousal.

They know each other or want to get to know one another and there is the prospect of a future relationship. Conversely, a hook up is a casual sexual encounter, which usually occurs between people who are strangers or brief acquaintances.
For instance, two people meet at a party where they have been drinking; they flirt and engage in sexual behaviours from kissing to sexual intercourse, with no commitment to a future relationship.
Bradshaw and team exposed 150 female and 71 male college students from a southern, public American university to a variety of dating and/or hooking up situations, such as when there was potential for a relationship, when their partner had a great personality and when drinking was involved.
Even though men initiated significantly more first dates than women, there was no gender difference in the number of first dates or number of hook-ups. For both men and women, the number of hook ups was nearly double the number of first dates.
Overall, both genders showed a preference for traditional dating over hooking up. However, of those students who strongly preferred traditional dating, there were significantly more women than men (41 percent versus 20 percent).
Of those who showed a strong preference for hooking up, there were far fewer women than men (2 percent versus 17 percent).
However, context mattered: when considering the possibility of a long-term relationship, both women and men preferred dating over hooking up; however, when the possibility of a relationship was not mentioned, men preferred hooking up and women preferred dating.
On the whole, men and women agreed on the benefits and risks of dating and hooking up. However, women seem to want a relationship more than men, says a James Madison release.
They fear, both in dating and hooking up, that they will become emotionally attached to a partner who is not interested in them.
Men seem to value independence more than women. They fear that even in hooking up relationships, which are supposed to be free of commitments, a woman might seek to establish a relationship.

Pfizer sues to block generic Viagra

Pfizer Inc. sued to prevent Teva Pharmaceutical Industries Ltd. from selling a generic version of the Viagra impotence drug.

Teva, the world’s biggest maker of generic medicines, is seeking U.S. Food and Drug Administration approval to market a copy of the drug, whose chemical name is sildenafil citrate. Pfizer, in a complaint filed March 24, contends the copy would infringe a patent that expires in October 2019.

Teva, based in Petah Tikva, Israel, has received tentative approval to sell a copy of the medicine, and plans to begin sales when another patent on Viagra expires in March 2012, Pfizer said in the complaint. The treatment for erectile dysfunction generated $1.89 billion in sales last year for Pfizer, including $962 million in the U.S., making it the company’s fifth-biggest drug.

Pfizer filed the complaint in federal court in Alexandria, Va. Denise Bradley, a spokeswoman for Teva, said the company had no comment on the new complaint.

It was a Super Bowl commercial that raised eyebrows around the country.
Six years ago, Eli Lilly and Co. introduced its new erectile dysfunction drug to a national audience with a 60-second spot that showed middle-aged and elderly couples snuggling and flirting, and relaxing in side-by-side bathtubs at sunset. A voiceover said: "Cialis. Are you ready?"
Then, in a line that would be the parodied by late-night comedians, the announcer rattled off a long list of possible side effects, including erections lasting longer than four hours.

Despite the complaints, Cialis quickly became a household name, and demand remains so strong that Indianapolis-based Lilly has been able to double the price of the drug since its launch in 2003, to about $16 a pill. Last year alone, during a recession, Lilly increased the price of Cialis twice. Competitors, including Pfizer (maker of Viagra) and GlaxoSmithKline (maker of Levitra), also raised prices.
But now Lilly faces new challenges to keep sales robust. Several other companies are developing new ED drugs, and some analysts think at least one will hit the market this year or next. Men should not underestimate the exercise element of regular sexual activity. Having sex burns off a lot of calories, increases the heart rate and is a good form of cardio exercise which is comparable to a good workout. The researchers said that their study could be useful for doctors as an indicator of potential heart disease. They said that asking patients about sexual activity could be “clinically useful”. The other thing which was observed was that men who get regular sex are more likely to be in an intimate and healthy relationship which in itself provides lower stress levels and general well being.

The FDA warning about the 'Stiff Nights' pill follows its investigations initiated after receiving a customer complaint about the product. 'Viagra effect' undermining Brazil's pension system

According to the INSS report, two out of three men who are separated remarry, while only one out of three separated women find a new husband.

Of the separated men, 64 percent of those aged over 50 remarry women younger than them. In the 60-64 age range, the proportion is 69 percent.

And the marked preference is for women aged 30 years younger.

Brazil has a mixed public-private pensions system.

Those in the public system receive the equivalent of their salary after retirement, while those with private funds receive a maximum of 1,800 dollars a month.

Under current laws, when a retired man dies, his wife continues to receive his full pension until her own death.

According to the INSS, 94 percent of pensions go to women.

"This is a grave and serious challenge for the future of the country, and it's going to require a reform of the pension system," Tafner said.

Like most fakes, the drugs seized were also named to confuse with the original ones. They were named Ramagra and Kamgra, which are fake versions of Viagra. Energy tonics and Kamagra jelly were seized from the godowns of Dhanlaxmi Transport. The FDCA team had carried out extensive raids at Sarkhej, Aslali, Sarangpur and Chhatral.

Over 18 lakh tablets of Ramagra, Kamagra and oral jelly were confiscated. Though the officials could not trace the origin of the drugs, they were supposed to be collected by Sujal Patel, who did not turn up to claim the consignment.

"Samples of the tablets seized have been sent to the laboratory for testing to ascertain the compounds used. Efforts are also on to trace the origin of the consignment to plug the influx of fake drugs into the market," said an official.

9 arrested for selling fake Viagra

RCMP in Quebec have arrested nine people and charged them with selling counterfeit Viagra pills over the internet and in sex shops.

It was the first operation in Quebec targeting counterfeit medication, which this is a growing problem, said RCMP Sgt. Noël St-Hilaire.

"If you've followed in the last year, different seizures made by different police forces in different areas where they're producing ecstasy, now they're producing Viagra," St-Hilaire said.

He said heart medication, cholesterol medication, even pills for psychiatric illnesses have been faked and sold on the internet for cheaper than the real thing.

Health Canada's Sylvie Cantin says counterfeit drugs can be made from anything.

"Those are dangerous … as a starting point, because the consumer has absolutely no information, assurance of safety, or how the product is made," Cantin said.

Cantin says Health Canada is trying to do more to stop fake pills from reaching consumers.

"We're very worried about the trend. We're worried because of the risks that go with it," he said.

Cantin said people should play it safe and get a prescription from a doctor and get it filled by a recognized pharmacist.

Each generation likes to think they invented sex, conveniently forgetting how they came into this world in the first place. By middle age, men’s sexuality becomes something that is infrequently discussed.
The medical profession is as much to blame. Only last week it was revealed that NHS Sheffield has produced leaflets reminding youngster of the enjoyable nature of sex and its health benefits. Under the slogan “an orgasm a day keeps the doctor away” (really, you couldn’t make this up), it is meant to update sex education by telling students about the benefits of enjoyable sex. And the NHS in Medway, Kent, have even started producing sex advice to pensioners.
Yet there’s an assumption that men in their 40s and 50s don’t need help in this regard. They are now in control of their lives and this includes their sex lives. For the busy, stressed, middle-aged man, trying to juggle a career, relationships and increasingly, support ageing parents, this should provide relaxation and enjoyment, but is, in fact, the very thing that can suffer.

Stress is well known to have a detrimental impact on libido and sexual performance. In recent years, research has shown the benefits of a sex life – from improving mood, regulating sleep patterns, helping the cardiovascular system and boosting the immune system.

Men are notoriously bad at seeking medical help, and even more so for sexual problems. Although this is an area with high success rates, it often goes untreated.

As men age, they are at increased risk of sexual dysfunction because of changes to circulation due to narrowing of the arteries, prostate problems and surgery, other problems such as diabetes or as a side effect of medication.

Drugs such as Viagra and Cialis have benefit in 70-80 per cent of men using them. Along with psychological treatments and surgery, success rates are over 90 per cent. Impotence can also be an early warning sign for conditions such as liver disease or heart and circulation problems.

But as men age, their relationships also change. Divorce and bereavement mean they can find themselves single again and may consider entering a new sexual relationship. Testosterone levels decrease from the age of 40 by about 0.5 per cent a year, and this can result in low libido.

While some men are happy to have sex infrequently, for others this causes problems yet can be readily diagnosed from a simple blood test.

The three best things a man can do himself to improve his sex life is to stop smoking, lose weight and reduce alcohol intake. Contrary to popular belief, some research also suggests that reducing red meat intake can improve libido, as can avoiding products naturally high in oestrogen-like compounds, such as soya.

The number-one rule, though, if you are not satisfied with your sex life, is talk to your GP. People’s sex lives do change as they age, but this doesn’t mean they cease to exist. Evidence suggests that those who engage in regular sexual activity live longer than those who abstain, even reducing the risk of prostate cancer in later life. Perhaps an orgasm a day really does keep the doctor away.

A recent survey is debunking the myth that one’s sex life dries up with age.

The survey found that many Canadians over the age of 50 report having adventurous, frisky — and even “ferocious” — sex as they get older.

As kids leave home and people move toward retirement, life stresses often ease up and inhibitions are shed, all of which makes for a renewed interest in sex.

“On the positive side, it looks like a lot of people are more adventurous. They say the quality of their sexual activity is better, and they have less inhibition,” said Dr. Stacy Elliott, director of the British Columbia Centre for Sexual Medicine at Vancouver General Hospital. “It’s refreshing to see something out there that’s positive about sexuality and aging.”

Nearly half of the people surveyed said their sexual inhibitions have decreased since they turned 50, and more than a quarter said the quality of their sex life has improved since turning 50.

More than half of the respondents said they wished their sex life was more spontaneous.

Elliott said the survey results are an accurate reflection of what she sees in her practice, where she sees many men who are interested in improving their sex lives, but also suffer from erectile dysfunction.

It is estimated that one in three Canadian men over the age of 40 suffer from some degree of erectile dysfunction, but Elliott said treatments have improved drastically in the past decade or so.

In her practice, treatments include medication, vacuum devices and penile injections. The oral medications were first introduced to the market in 1998 and have improved since then to include a daily pill that a man can take with his morning vitamins, rather than in the heat of the moment.

“The daily pill dissociates being sexual with taking a pill,” Elliott said. “And it means that you’re always prepared, so that’s a big factor in spontaneity and freedom as men get older, instead of planning everything out to a tee.”

The survey, which was conducted online between April 14 and April 20, was sponsored by Eli Lilly Canada, the maker of a daily pill to treat erectile dysfunction. Interviewed were 1,510 respondents over the age of 18, 494 of them older than 50. The margin of error for the respondents over 50 is plus or minus 4.4 percentage points, 19 times out of 20.

Elliott described the renewed interest in sex among mid-lifers as a “reinvigoration,” but emphasized that it’s not universal among all people over 50.

“I think if you’re really bushwhacked from life’s stressors, sex doesn’t necessarily percolate to the top of your priority list,” she said.

But if sex is not a personal priority or concern, Elliott said there is no reason to worry.

“I don’t want people to think, ‘I’m not sexual. What’s the matter with me?’ If they’re happy with that, that’s fine.”

A big reason Viagra - the little blue pill launched in 1998 - and the other two brands that followed became best-sellers is that ED affects many men.

Dr Gan Tek Kah from Singapore Men's Health Clinic said: 'Statistics show that 50 per cent of men above 50 will have some form of ED, from mild to moderate to severe.'

Since 1998, Pfizer has sold an estimated 1.8 billion of its Viagra pills to 35 million men worldwide. More than 3.5 million pills have been sold here so far.

Levitral's manufacturer Bayer declined to reveal figures but said sales in Singapore had 'increased over the years since its launch in 2003'.

Cialis' spokesman could not be reached for comment. The drug was launched in 2003.

Doctors say the drugs benefit men with certain medication conditions. They also help men who, as they age, find their lowered level of testosterone driving down their sexual drive or libido.

Mr T.K Lim, 59, for example, found that his diabetes kept him from achieving an erection. So he went to a doctor.

'Viagra saved my life,' said the married clerk, who had first tried herbal concoctions. 'It was the only thing that worked.'

Dr Gan said he has seen a 30 per cent increase in prescription of these pills at his clinic, compared to five years ago.

Professor Peter Lim, senior consultant urologist and president of the Society for Men's Health Singapore, said there has been a 400 per cent to 500 per cent hike in demand, compared to the late 1990s.

Prof Lim said he even sees more patients aged 80 or older coming forward.

The doctors contacted also said more young men with anxiety issues or stress are turning to these pills. Dr Michael Wong, a senior consultant urologist, said: 'A big group is newly-weds who can't perform because expectations are too high.'

Then there are the 'young studs' who want enhanced sexual pleasure. They see Viagra and the other brands as sex pills.

Will such regular pill-popping give rise to addiction?

Doctors said the three prescription brands are not known to be physically addictive.

But they added that the issue of 'psychological addiction' can arise.

'People could strongly associate the medication with sexual pleasure,' said psychiatrist and sex therapist Calvin Fones. 'There might be an exaggerated reliance on them and the users feel they cannot do without them.'

These people, Dr Fones said, 'give the tablets too much credit' and pop them each time they engage in sex.

Sex addicts, usually men, use sex as a form of release from stress and self-esteem issues at work and in their marriage. 'A number of these sex addicts use the pills to keep going,' said Dr Wang.

Prof Lim said those addicted to such pills tend to be younger men who want to enhance their staying power so they can have many sex sessions a night.

'They want to keep doing it and could well use a higher dosage each time,' he said. 'This is psychological addiction.'

The doctors felt that such pills should be prescribed only to men with ED issues. But these pills may be easy to come by, as John found.

Also, many of those with addiction issues tend to get their supply online, over the counter overseas or in Desker Road and Geylang.

But over the past 17 months, 10 Singaporeans have died from taking illegal, often fake, sex pills.

The doctors said abuse of ED drugs could lead to men paying for commercial sex and thus exposing themselves to HIV risks through sex with multiple partners.

The drugs may also cause marital woes resulting from men's infidelity and tension between couples as a result of a change in sexual habits and patterns.

'A lot of the time, sexual response has to do with self-confidence and psychological issues,' said Dr Fones. 'Men who enhance their confidence instead will be surprised to find they can have satisfying and enjoyable sex without the pills,' he added.

Methods. A systematic review of the literature regarding the aforementioned issues.

Risk factor-associated cavernosal endothelial dysfunction is mostly induced by unifying mechanisms, including oxidative stress and impaired endothelial nitric oxide functional activities, which present clinically as ED. Several techniques to evaluate endothelial dysfunction were revised, with advantages and limitations debated, focusing on our detailed expertise using the PNORT method.

Over many years Hackett reports regularly seeing patients referred with erectile dysfunction after a heart attack, only to hear that they had developed erectile dysfunction two to three years before—a warning sign ignored by their general practitioners.

It is well known that erectile dysfunction (a symptom of vascular disease in the smaller arteries) doubles the risk of heart disease, a risk equivalent to being a moderate smoker or having an immediate family history of heart disease. Erectile dysfunction in type 2 diabetes has been shown to be a better predictor of the risk of heart disease than high blood pressure or high cholesterol.

But despite this considerable evidence erectile dysfunction is still treated as a recreational or "lifestyle issue" rather than a predictor of a serious health problem, says Hackett.

The UK government has pledged to reduce the death rate from coronary heart disease and stroke and related diseases in people under 75 by at least 40% by 2010, yet there is no screening for erectile dysfunction in patients with diabetes or heart disease, he says.

"Continuing to ignore these issues on the basis that cardiologists feel uncomfortable mentioning the word 'erection' to their patients or that they may have to deal with the management of a positive response, is no longer acceptable and possibly, based on current evidence, clinically negligent", he concludes.

Analyzing a five-year study of 989 men aged 55 to 75 years from Pirkanmaa, Finland, the investigators observed that men reporting intercourse less than once per week at baseline had twice the incidence of erectile dysfunction compared with those reporting intercourse once per week. Erectile dysfunction incidence was 79 cases per 1000 in men who had reported sexual intercourse less than once per week, dropping to 32 cases per 1000 in men reporting intercourse once per week and falling further to 16 per 1000 in those reporting intercourse 3 or more times per week.

In addition, the frequency of morning erections predicted the development of complete erectile dysfunction, with an approximate 2.5-fold risk among those with less than 1 morning erection per week compared with 2 to 3 morning erections per week

Writing in the article, Juha Koskimäki, MD, PhD, Tampere University Hospital, Department of Urology, Tampere, Finland, states; "Regular intercourse has an important role in preserving erectile function among elderly men, whereas morning erection does not exert a similar effect. Continued sexual activity decreases the incidence of erectile dysfunction in direct proportion to coital frequency."

'Viagra coffee' seized

Malaysia's health authorities have seized over 20,000 dollars worth of coffee mixed with sildenafil, the main ingredient in erectile dysfunction drug Viagra, a report said Sunday.

"The coffee mixture had been distributed in sachets nationwide," an official was quoted as saying by the New Sunday Times newspaper.

"Officers found 900 boxes containing more than 9,000 sachets of the coffee mixture which retailed for more than 80 ringgit (23 dollars) per box," the officer added without naming the coffee brand in question.

Enforcement officials said the coffee was mixed with sildenafil to "perk up" drinkers beyond the usual stimulation provided by caffeine.

Health authorities recently raided the distribution company located in Gombak, just north of the capital Kuala Lumpur, the report said.

It cited a health ministry official as saying the company used individuals to distribute the product in small quantities to evade detection by enforcement agencies.

"Investigations revealed that the distributor attempted to mislead the public by claiming that the coffee mixture could provide an energy boost, besides being beneficial for men," he added.

Two years ago Malaysia made its largest single drug bust when it seized at least four million dollars-worth of a fake erectile dysfunction treatment labelled "Miagra".

The counterfeit pills, closely resembling the blue pills of the genuine Viagra brand, were destined for Malaysian and Thai markets.

According to various studies, alcohol, tobacco, performance anxiety, depression and stress could aggravate the problem. Many common medicines including blood pressure drugs, antihistamines, antidepressants, tranquilizers, appetite suppressants and cimetidine (an ulcer drug) could cause erectile dysfunction (ED) as a side effect. Emotional stress like depression, anxiety, failed relationship, guilt, mental stress or exhaustion lead to addiction either to drugs, alcohol or smoking, and all these are known factors for impotency, the doctor said.

Emotional factors affecting sex and men’s ability include interpersonal issues that relate to relationships and psychological problems.

Between 35 to 50% men with diabetes experience ED, Kumar said. Also, surgery, especially radical prostate and bladder surgery for cancer, could injure nerves and arteries near the penis, causing the condition. Injury to the penis, spinal cord, prostate, bladder, and pelvis are some other causes.

Eating for a healthy prostate

If you're part of the cohort of ageing male boomers, then there is a good chance you were up last night to go to the bathroom - maybe several times. With ageing comes a gradual enlargement of the prostate gland, which can press on the urethra, ultimately restricting urine flow - or even blocking it entirely. This condition, known as benign prostatic hyperplasia, or BPH, is very common in older men, and while it may be relatively harmless for some, it can also be a precursor for prostate cancer, and therefore should be monitored carefully by your physician.

From a dietary perspective, there haven't been enough well-controlled studies on any nutrients or supplements to be able to draw definitive conclusions, but there is evidence that nutrients such as lycopene (a component released when tomatoes are cooked), the herbal supplement saw palmetto (at a dose of around 500 mg per day), ground flaxseed (two tablespoons per day), soy foods and a low-fat diet can all keep levels of prostate specific antigen (PSA) - a marker of prostatic growth under control. Less convincing is the evidence that taking vitamin E or selenium can prevent prostate cancer; in fact, the large SELECT study, examining the link between both supplements and the development of prostate cancer, was halted last year after it failed to produce any significant results.

Diet and erectile dysfunction

The path by which one develops erectile dysfunction actually has many of the same points of origin as heart disease, which has resulted in some researchers calling ED something of a harbinger of cardiovascular disease. In other words, if the plumbing isn't working down there, it could be a sign it's not working somewhere else, too.

Taking a pill to manage ED is one option, but the best bet is to also take a serious look at diet and lifestyle. Researchers in Italy have demonstrated that losing a bit of weight (about 10% of your initial weight, or about 20 lbs on a 200 lb man) and adopting a Mediterranean-type diet, can improve sexual function in about one-third of men with ED, without the use of medications. Need a refresher on the Mediterranean diet? Here are the key components:
-Choose fish or lean cuts of white meat, like chicken or turkey, more often. Consume red meats sparingly (preferably no more than once or twice per week), and when you do, choose lean cuts.
-Consume plenty of green vegetables and fruits, especially berries.
-Get healthy fats from nuts, olives, olive oil and avocados on a daily basis.
-Choose whole grains over white or refined grains.
-Dairy products are OK, especially yogourt. Eat cheese in moderation.
-And finally, don't forget the legumes, such as chick peas (think: hummus).

While eating this type of diet might be a big change for some, there is much to be gained from slow, steady progress. That might mean adding ground flaxseed to your cereal for prostate health, or choosing yogourt with nuts and berries for breakfast instead of a bagel with cream cheese. It turns out that eating this way can not only help keep your waistline stable and your heart healthy, it might also revitalize your love life, too. And that's good news for men and women alike.

The study which was written up in the medical journal BJU International looked at 40 teenage boys between the ages of fourteen and nineteen who were treated at specialist erectile dysfunction clinics. The boys tended to have suffered from their problem for a couple of years and usually were accompanied by one or both of their parents.
Twenty five of the boys underwent ultrasound scanning to examine penile blood flow and from this twelve of them were diagnosed as having blood vessel problems. The others were therefore thought to have a psychological cause to their erection problem. Because of the age of the boys not all of them were sexually active but some of them had become increasingly concerned that they did not wake up with morning erections like their friends.
Doctor Mulhall noted that that in the majority of cases of teenage erectile dysfunction the treatment was effective whether the condition was caused by psychological or physical reasons. When the cause of erectile dysfunction was diagnosed as physical treatment often involved vascular surgery or hormone therapy.
When the condition was psychological which could be due to lack of confidence or distressing previous sexual experiences, medications along with psychotherapy were used. He said that once the boys had regained their sexual confidence they would not need to use the medication again.
Doctor Mulhall also commented on the extreme stress that teenage boys who were unable to get erections suffered from and how embarrassing and humiliating it could be for them even to discuss it with their parents or a doctor. He said that research like this which showed that teenage erectile dysfunction was almost always curable would encourage teenagers to come forward and get help.

12-month-old Alfie Oliver, was born last July with a narrowing of blood vessels around his lungs and was given just weeks to live after suffering a cardiac arrest.
Parents Tracey and Rob Oliver were plunged into despair by the diagnosis, and Alfie was forced to endure a series of gruelling heart operations when he was just days old.
But after a team of experts from Great Ormond Street Hospital heard of his story they suggested the blue pills could open up his blood vessels and improve circulation.
Now Alfie's family are preparing for the first birthday they thought they would never see as he continues taking his Viagra.
And because of his young age the baby is not experiencing the notorious side-effects of the drug.

High street access to Viagra given green light

The erectile dysfunction drug Viagra was last week made available on the high street for the first time, under a scheme launched by the pharmacy chain Alliance Boots.
Viagra will be made available in 29 stores across England and Wales. It will be dispensed without a prescription under a patient group direction (PGD), following a successful pilot trial in Manchester in 2007, involving 288 men.
The initial prescription will cost £55 with subsequent doses priced at £26.59.
Patients will only be sold the drug after completing a 30-minute consultation with a pharmacist, which includes fill-ing in a questionnaire.
Patients must also be regis-tered with a GP and must agree to their GP being informed about their Viagra use.
Previously, GPs have warned that making Viagra available on the high street would mean that fewer men would visit their GP, allowing any underlying erectile dysfunction problems to go unnoticed.

Pfizer Wins Canadian Patent Protection for Viagra

Pfizer Inc., the world’s biggest drugmaker, has a valid Canadian patent on the impotence pill Viagra, a federal judge ruled, dismissing a bid by Novopharm Ltd. to introduce a generic version of the drug.
Judge Michael Kelen, in a 54-page ruling today, prohibited the Canadian minister of health from approving a generic version of Viagra until New York-based Pfizer’s patent expires in 2014.
Novopharm, a unit of Petah Tikva, Israel-based Teva Pharmaceutical Industries Ltd., argued that Pfizer’s patent was invalid because it was obvious. Kelen rejected the argument saying it wasn’t obvious in 1994, when Pfizer first developed the treatment, that sildenafil, the main ingredient in Viagra, would be effective in treating erectile dysfunction.
“It was the ‘holy grail’ of impotence therapy,” Kelen wrote in today’s ruling. “When it was finally learned that Pfizer had developed sildenafil for the treatment of impotence, experts wrote this was a ‘revolutionary concept.’”
The ruling is subject to appeal to the Federal Court of Appeal.
“We believe intellectual property protection is vital to supporting the enormous investments required to develop life- saving new medicines,” Pfizer said in an e-mailed statement. “We will continue to take appropriate action to defend our intellectual-property rights.”
Novopharm’s lawyer, Marcus Klee, didn’t immediately respond to a request for comment.
Information Lacking
Novopharm also claimed Pfizer’s patent was invalid because it doesn’t provide enough information about the invention. The patent covers 260 quintillion -- a quintillion in the U.S. and Canada is a one followed by 18 zeros -- compounds, including compounds that haven’t been found effective in treating erectile dysfunction.
The judge said he must abide by precedents that have allowed companies to patent classes of compounds. The patent shouldn’t be invalidated by such an objection, 13 years after it was opened for public inspection, Kelen wrote.
The judge criticized the practice in an aside to the verdict, writing of his “discomfort with the existing jurisprudence” that allows patents to be described in such a way that “the skilled reader must undertake a minor research project to determine which claim is the true invention.”

United Therapeutics Jumps on Expanded Cialis Approval

“We have reviewed the label and it looks to be a near best-case scenario,” Mark Schoenebaum, an analyst at Deutsche Bank in New York, said in a note to clients today. “In a ‘best- case’ scenario, we believe Adcirca U.S. sales could reach $200 million before the 2017 patent expiration.”
Lilly, of Indianapolis, sold U.S. rights for the pulmonary arterial hypertension use to United Therapeutics in November. Other drugs used to relax blood vessels include Pfizer Inc.’s Revatio, Actelion Ltd.’s Tracleer and Gilead Sciences Inc.’s Letairis.

Canadian researchers say the prescription drug, Viagra, is as good for the heart as it is for the love life. Seems the way the drug influences the platelets in the bloodstream also minimizes the risk of heart attack and stroke.

Now, however, Donald Maurice, professor of pharmacology and toxicology at Queen’s University, Kingston, Ontario, has discovered an enzyme in the drug which targets platelet activity, too, and suggests it does so in a way that may prevent massive and sudden heart attacks or strokes in people who’ve had arterial stents implanted.
Platelets are a natural part of the blood’s system of clotting. They cling together to stop the flow of blood at the site of injury so a small wound doesn’t become life threatening. When stents are implanted, platelets have a tendency to cling to the stents and can eventually form a complete blockage of the stented artery. When this happens, deadly heart attacks and strokes can occur with little or no warning.
Each cell has two different clusters of an enzyme identified as phosphodiesterase type 5 (PDE5) although only one cluster affects platelet function. Viagra inhibits the production of PDE5 but figuring out how to manipulate that enzymatic production has remained elusive, until the Queen’s researchers made the breakthrough discovery that only one cluster of PDE5 in each cell affects platelet activity.
With better understanding of cell function in relation to the PDE5 enzyme, the research team expects to develop a way to manipulate just one cluster of PDE5 while leaving the other cluster intact. Viagra and similar drugs that inhibit PDE5 are at the forefront of future studies of this nature.

VIAGRA’S OTHER TALENTS: TO HELP A ‘SIGNALING’ PROTEIN SHIELD THE HEART FROM HIGH BLOOD PRESSURE DAMAGE

Johns Hopkins and other researchers report what is believed to be the first direct evidence in lab animals that the erectile dysfunction drug sildenafil amplifies the effects of a heart-protective protein.
The team’s findings, to be published in the Journal of Clinical Investigation online Jan. 5, helps explain why sildenafil, more widely known as Viagra, has already been shown to improve heart function and may one day have value in either treating or preventing heart damage due to chronic high blood pressure.
The key, investigators say, is sildenafil’s effects on a single protein, RGS2, newly identified in the latest study as an essential link in the chain reactions that initially protect the body’s main blood-pumping organ from spiraling into heart failure.
Experimenting in mice, the team of heart experts first established that after a week of induced high blood pressure, the hearts of animals engineered to lack RGS2, or regulator of G-protein signaling 2, quickly expanded in weight by 90 percent. Almost half the mice died of heart failure. In mice with RGS2, by contrast, the dangerous muscle expansion, known as hypertrophy, was delayed, growing only 30 percent, and no mice died.
Subsequent tests treating hypertensive mice that had RGS2 with sildenafil showed enhanced buffering, with less hypertrophy, stronger heart muscle contraction and relaxation, and as much as 10 times lower stress-related enzyme activity compared to their untreated counterparts. In mice lacking RGS2, sildenafil had no effect.
“Sildenafil clearly prolongs the protective effects of RGS2 in mouse hearts,” says study senior investigator and cardiologist David Kass, M.D.
According to Kass, a professor at the Johns Hopkins University School of Medicine and its Heart and Vascular Institute, RGS2 is stimulated by an enzyme, protein kinase G, whose action is, in turn, raised by countering the activity of another enzyme, phosphodiesterase 5 (PDE5A). Sildenafil’s ability to block PDE5A was shown by Kass and his team in 2005 to be responsible for blunting hypertrophy due to high blood pressure in mice and offsetting similar, adrenaline-stimulated heart stress in people.
Kass says RGS2 “acts like a short-term reset mechanism in the heart,” recoupling G proteins that if left alone stimulate the heart’s response to high blood pressure. And without the “reset,” a cascade of reactions known as Gq signaling leads to scar tissue formation, hypertrophy and heart failure.
Currently, physicians use so-called ACE inhibitor and ARB inhibitor drugs to block Gq signaling. These classes of drugs are the most common treatment for heart failure, which afflicts more than 5 million Americans, killing over a quarter million of them each year.
“The evidence is piling up that unbridled Gq signaling is driving a central biological chain reaction in heart failure,” says Kass, “and that by extending the protective effects of RGS2 or by developing a test for its presence, researchers can develop new therapies or improve existing ones, including ACE inhibitors and possibly sildenafil, for people with heart failure who will benefit most.”
Until recently, scientists thought RGS proteins, which are found only in small quantities in the heart -- a thousand times less than other, more common proteins, such as myosin and metabolic proteins -- played no key role in heart function. Previous tests in mice, Kass says, had shown no harmful effects to the heart from knocking out production of RGS2, though the protein was known to have a role in maintaining smooth muscle function in blood vessels.
But studies by co-investigators at Tufts Medical Center in Boston had shown that RGS2 activity was upped by protein kinase G, leading Kass and others to look for stronger links between these biological pathways and hypertrophy.
The latest study involved more than a half-dozen experiments, all performed within the last three years and designed to zero in on the role played by RGS2 in stalling hypertrophy.
In one experiment for the current study, researchers artificially stimulated the Gq chemical pathway in mice lacking RGS2, worsening the effects of Gq signaling, including hypertrophy and widened heart chambers.
In another experiment in mice with and without RGS2, researchers analyzed the cardiac response to the physical stress of twice daily swimming exercises lasting 90 minutes each, a stress not known to affect Gq signaling. After six weeks of testing, both sets of mice showed similar increases, at 30 percent, in heart mass and no signs of impaired heart function.
Subsequent protein analysis for enzymatic action common to heart failure showed the same results for both sets of mice, confirming to researchers that RGS2 proteins were responsible for protecting the heart from hypertrophy linked to Gq signaling.
More tests with pressure overload showed that when RGS2 was stimulated by protein kinase G, both proteins moved together from inside the cell to its outer cell walls. This effect was then stabilized in RGS2 mice treated with sildenafil, solidifying evidence of the biological chain reactions between the drug and the protein.
“Our results offer among the first insights into the biology of the RGS2 protein in heart cells during hypertrophy,” says study lead investigator Eiki Takimoto, M.D., Ph.D. “This greatly expands our understanding of how high blood pressure affects the heart and helps break down the disease equation into its molecular components for subsequent clinical testing.”
Takimoto, an assistant professor at Hopkins, says the team’s next plans are to look at other potential consequences of increased RGS2 activity within the cell and to zero in on what other proteins or factors boost its action.
PDE5A is involved in the breakdown of a key molecule, cyclic guanosine monophosphate, which helps control stresses and limit overgrowth in the heart. PDE5A is also the biological pathway blocked in the penis by sildenafil to promote the relaxation of blood vessels and maintain erections.
Funding for the reported study was provided by the National Institutes of Health, the Peter Belfer Laboratory Foundation, and the American Heart Association.

Sikkim to market Himalayan Viagra

The Sikkim government is considering a proposal by the Forest and Wildlife Department to market the medicinal herb Cordyceps Sinensis popularly known as Himalayan Viagra.
The herb, whose scientific name is Caterpillar Fungus, is found abundantly in the forests of the Himalayan state.
Apparently taking cue from the neighbouring countries of Bhutan and Nepal which have legalized the trade of the Cordyceps Sinensis herb, the Forest and Wildlife Department of the state government has also proposed a similar measure by the state.
The department's officials argued that since the herb's availability in the black market fattened the pockets of only a handful of smugglers, its sale in the open market would benefit both the Forest and Wildlife Department as well as the local people.
They also cited the widespread illicit trade of forest produce to buttress their argument in favour of the open market sale.
The department also suggested that a regulated mechanism for plucking the herb from the forests be put in place and authorized dealers be appointed to market it, official sources told PTI here.
The Himalayan state could generate a significant amount of revenue from marketing the medicinal plant which costs between 2000 to 2600 US dollars per kg in the open market, the sources said.
The draft proposal has suggested a mechanism for collection and marketing of the herb under which only the Joint Forest Management Committees (JFMCS), registered with the forest department, will be allowed to collect it.
The only condition is that the jfmc will have to be from the particular area where the herb is found.
The collection of herbs from the wildlife and forest sanctuaries will, however, be not permitted under any circumstances in view of the sanctity of the reserved forests, they said.
The draft proposal has also suggested a formula for sharing of revenue from the marketing of the medicinal herb on the basis of 25:75 to the forest deparment and JFMCS, the sources said.
It may be mentioned that the valued medicinal herb is found in vast forest areas in East, West and North districts of Sikkim at places like Green Lake, Dzongri, Thamjey, Nathu La pass, Doman Valley, Tholung pass, etc, located above 8,000 feet under the jurisdiction of the forest department.
Though the marketing of the medicinal plant is banned in Sikkim, it is available in the black market for years with smugglers making crores of rupees by selling it to shady dealers both Indian and foreign, the department's officials said.
Recently forest department sleuths had nabbed two smugglers with a haul of 17 kg of the Himalayan Viagra from Singtam in July, last year.

New Use for Active Ingredient of Cialis

The FDA has approved a new use for tadalafil, which is the active ingredient in the erectile dysfunction drug Cialis. Tadalafil, sold as Adcirca, is now approved to improve exercise ability in people with pulmonary arterial hypertension, which is a rare, life-threatening lung disorder that causes high blood pressure in the lungs.
Adcirca comes in 40 milligram tablets; patients take one tablet per day.

How will male General Motors retirees get their motor runnin'?
There's been a lot of buzz around the CNBC newsroom and a certain amount of banter on our air today regarding a nugget in this Wall Street Journal piece about GM retirees facing a hard choice.
Do they start paying for their impotence drugs out of their own pocket? Because apparently, under the new deal the company won't pick up the tab anymore.
GM has been the largest private purchaser of ED (erectile dysfunction) pills. The last published estimate I can find is from 2006 when GM reportedly bought $17 million worth of Pfizer's Viagra. When I asked a Pfizer spokesperson if that number's accurate I was told, "No comment." She read me a brief company statement in response to my inquiry about how many of the little blue pills PFE sells to GM: "We don't discuss our relationships with our customers."
But even if the $17 million from a few years ago is right, it's a drop in the bucket. Pfizer sold nearly $2 billion worth of Viagra last year. Eli Lilly sold $1.4 billion worth of Cialis. Bayer, GlaxoSmithKline and Schering-Plough share Levitra, but the sales numbers are very low. So, even if you were to assume a worst-case scenario where all of those GM ED sales would go away, it wouldn't be financially material for the companies. Still, these days the big drug companies need to hang onto every little bit of revenue they can.
Now, the cost to the mens' relationships is another story.
But at least one drug industry analyst agrees with me that the impotence pills will be one of the last things the guys will decide to go without. Yeah, they might cut more of them in half, but I don't think they're going to stop buying them.
Maybe Pfizer and Lilly should consider doing a special GM retiree rebate program to try to make sure they don't lose the business.

Opening of sexual health clinic delayed due to budget constraints

A sexual health clinic for youth originally planned to open in Oakville this year has been postponed due to regional council’s recent direction to staff to bring in a budget with no tax hikes in 2010, according to a report tabled at yesterday’s health and social services committee.

Council will review the report at its meeting June 3.

This year’s budget had included $311,000 to open the clinic, which would treat sexually transmitted diseases, including the increasingly prevalent Chlamydia, as well as providing immediate, anonymous HIV testing and pregnancy testing and counselling.

The new clinic could have helped alleviate the current three-week wait times at the over-capacity south Halton clinic in Burlington and provided a more appropriate and accessible location for older teenagers in Oakville, said Dr. Bob Nosal, Halton’s Medical Officer of Health.

“Once council approved the motion for the freeze in staffing (levels) and the tax freeze, that program (the Oakville clinic) along with other program changes basically came off the table,” said Nosal. “And it won’t happen next year because, with the budget freeze, there won’t be staff.”

Nosal said the public health department, which also runs such harm prevention clinics in Milton, Acton and Georgetown, would manage with the resources provided, considering the current economic downturn. However, he admitted the lack of a clinic in Oakville meant youth in that municipality might have nowhere to turn if they don’t feel comfortable visiting their family doctors, the original reason the clinics were first opened in 1975.

“They will not be served, that is the reality,” said Nosal.

Halton regional chair Gary Carr, who initiated the drive to achieve a tax freeze next year, said the clinic could still open in 2010 if council decides to shift some priorities.

The staff report outlining budget directions for 2010 stated there would not be cuts to existing service levels. The delay in opening a sexual health clinic is not viewed as a cut to services as it is considered a new initiative.

Sexual health kiosks support vulnerable teenagers

New sexual health advice and dispensing kiosks, to support vulnerable teenagers, are being rolled out by primary care trusts (PCTs) and local authorities.

Developed and made by Engage Now, the secure, touchscreen Medi+Vend kiosks are designed for installation in medical and youth centres, schools and colleges and other locations such as homeless shelters.

The interactive Medi+Vend kiosks allow patients clients to obtain free condoms, kits to test for diseases such as chlamydia and health advice and information on local sexual health services. So far, more than 80 of the kiosks have been sold to PCTs and similar organisations across the UK.
Recent Medi+Vend sales include 27 to local authorities and PCTs in London and the south east, 31 in Birmingham, Midlands and south west, and 28 in the north. This is in addition to sales of Engage Now’s interactive Drugs Box and Drink Angel kiosks, which offer drug and alcohol advice to teenagers.

The East Yorkshire company’s success with Medi+Vend was recently recognised with a regional health service trophy at the annual Service Improvement Day awards, where it won the Innovative Health & Social Care Technology Award.

Mal Williamson, managing director of Engage Now and creator of Medi+Vend, says the kiosks have been welcomed by young people and sexual health workers alike: “The Medi+Vend kiosks are about helping teenagers take control of their own lives, about them being responsible for their own actions, but on their terms, in their time, and in places where they feel at ease.

“Young people are often perceived as being out of control, but both the perception and actuality of that are frequently erroneous when all they sometimes need is help and guidance. Medi+Vend is one way in which they can be educated to look after themselves when it relates to sex.

Today's seniors, their sex lives liberated long ago by one pill and extended indefinitely by another, have every intention of staying in the game. The scope of the sexual shifts launched by Viagra a decade ago--perhaps as monumental as those triggered by the birth control pill--is now becoming apparent. A July study published in the British Medical Journal found that considerably more 70-year-olds are enjoying sex regularly than 30 years ago: 57 percent of men and 52 percent of women today versus 40 percent and 35 percent back then. (And that's before the baby boomers arrive with their outsize expectations.)

About one quarter of those ages 75 to 85 are now sexually active, according to recent research from the University of Chicago. "We used to think seniors didn't have sexuality at all," says Helen Fisher, a professor of anthropology at Rutgers University and author of Why We Love. "Viagra has caused many older people to put sex way up front in their relationships."

That could explain why U.S. sales of the little blue pill and two newer drugs have skyrocketed to more than $1.5 billion--or 19 million prescriptions--a year, causing drug companies to salivate over the possibility--still distant--of a "pink Viagra" for women. Further along in the pipeline: a testosterone gel to reverse libido loss after menopause and a pill that boosts desire by acting on serotonin receptors in the brain.

The merits of staying sexually active through the years are obvious and plentiful: joy and excitement, connectedness, and a host of health benefits. Scientists have shown that having sex regularly boosts the immune system and releases hormones that lower stress levels, improve sleep, and might even hold off wrinkles: A Scottish study found that people who enjoy sex every other day looked about seven to 12 years younger than their peers, on average.

Beyond drugs. But it's also clear that fulfillment depends on far more than pharmaceutical fixes. "Erectile dysfunction drugs are marketed to suggest that older men will get back the kinds of erections they had in their 20s," and that's simply not the case, says Barry McCarthy, a professor of psychology at American University in Washington, D.C., and author of Men's Sexual Health . Clinical trials show that about 65 to 85 percent of men with erectile dysfunction find their problem improves with Viagra or one of the other two ED drugs, Levitra or Cialis. Yet, one third of men responding to a recent Consumer Reports survey said they experienced side effects from the drugs like headaches, heartburn, or, in rarer cases, prolonged painful erections. And men tempted to try drugs just for a little boost will most likely be disappointed at what Viagra can't do. "It's not an aphrodisiac and won't make a man bigger," says Abraham Morgentaler, an associate clinical professor of urology at Harvard Medical School and author of The Viagra Myth. "It also won't make a guy's erection any better if he doesn't have a problem in the first place."

Prostate cancer treatments, which can permanently damage nerves that cause erections, present the biggest challenge in terms of robbing men of their sexuality. "At best, Viagra helps about half of those with no function," Morgentaler says. But perseverance can pay off. There isn't enough evidence to determine which erectile dysfunction drugs work best or cause the least harm, according to a report released Tuesday by the U.S. Agency for Healthcare Research and Quality.
